SmartCare360 Stroke Management is a virtual care program designed to help stroke survivors recover safely and effectively from home. We connect patients with neurologists, therapists, care coordinators, and educational resources to support physical, cognitive, and emotional healing.
Any adult who has experienced a stroke and qualifies for Medicare or Medicaid may be eligible. We also support patients with chronic stroke-related impairments, including difficulty with mobility, speech, memory, or daily functioning.
Services include remote neurologist consultations, virtual physical/occupational/speech therapy support, medication and risk factor monitoring, mental health counseling, caregiver coordination, and personalized recovery plans—all from the comfort of home.
After a virtual assessment, you’ll receive a custom care plan. You’ll then have access to scheduled telehealth appointments, remote monitoring tools, educational content, and ongoing support from our care team—all through our secure platform.
If you qualify, SmartCare360 provides or helps coordinate devices such as blood pressure monitors, tablets, or wearable sensors for remote monitoring. All you need is a basic internet connection—we’ll guide you through setup and usage.
